Transitions is looking for a creative, innovative and energetic person to become a Team Lead for Mobile / Residential Supports.

TA Team Lead is responsible for supporting the leadership team in all efforts to fulfill Transitions’ purpose, while modelling our Core Values: Diversity-Connection-Community-Teamwork-inclusion and Belonging.

Through ongoing commitment and coaching, the Team Lead assists Direct Support Professionals in aligning support with these values, fostering a positive and person-centered environment. The successful candidate will possess the value of equality and the skill set needed for community networking, and facilitation supports for multiple people. The applicant must be a creative thinker, multi-tasker, possess strong organization and problem solving skills and have the ability to oversee a caseload of up to twenty-five (25) staff.

The person must be able to work independently and within a flexible schedule, including providing weekend on-call support on a rotating basis (approximately once every five (5) weeks.

Hours of Work Staff work within a pre-determined rotation. Schedules are provided in advance; however, occasional shift adjustments may be necessary to meet with needs of the individuals or the program. Shift times include: 8a – 4:30p and 2:30p – 11p

JOB RESPONSIBILITIES:

  • Balancing multiple demands and responding to immediate needs.

Decision Making:

  • Scheduling of staff to meet the needs of persons supported.
  • Mitigating the day-to-day operational needs for service delivery.
  • Decision making as required when providing On-Call Supports.

Key Contacts:

  • Internal: Direct Support Professionals, Persons Supported, Parents/Guardians, Office Staff
  • ExternalCommunity Agencies and Partners, Employers, PDD-Disability Services Caseworkers.

Leadership and Support to Direct Support Professionals:

  • Understands and supports Transitions’ Vision and Mission statements.
  • Is knowledgeable about Transitions framework and has an understanding of the strategic/operational plans that guide the services we provide.
  • Is professional, honest and ethical at all times.
  • Coaches, supports and provides direction to team members.
  • Deals effectively with crisis; assessing the risk and acts accordingly.
  • Attends and participates in Team Lead meetings.
  • Completes staffing schedules and ensures staff have adequate training.
  • Chairs team meetings.
  • Identifies and shares with team members, resources and services within the community.
  • Identifies staffing needs with Supervisor and may be involved in the interviewing and selection process for staffing.
  • Oversees the in home orientation process.
  • Contributes and participates in team member’s performance appraisals.
  • Participates in the On-Call rotation (approximately once every 6 weeks).
  • Trained in all aspects of personal care needs and is able to teach and support staff in this area. Is able to provide personal care, if needed.
  • Complies with all workplace health and safety rules and guidelines. Completes Working Alone Safety Checks on staff as part of the Occupational Health and Safety Requirements
  • Identifies teams’ training needs with Supervisor and helps to create a plan to address the needs.
  • Other duties as assigned.

Qualifications:

  • Diploma in Disability Studies or related area of study. Previous direct or related experience may be considered equivalent.
  • A minimum of 2 years’ experience in the disability or related field of work.
  • Previous leadership experience is an asset.
  • Strong organizational, problem solving and interpersonal skills.
  • Ability to effectively communicate (both verbal and written).
  • Previous work experience in a not-for-profit organization would be an asset.
  • Must have a clear, current Criminal Record Check, which includes Vulnerable Sector.

Job Type: Full-time

Pay: $24.24-$31.90 per hour

Schedule:

  • Day shift
  • Evening shift
  • Monday to Friday
  • On call
  • Weekends as needed
